A gap is showing inside the entrance/exit of the Forsaken Cave.

This is a mesh issue. It's all one big piece on the cave entry, and it may affect other similar pieces in other styles.

Fix here. I reworked the geometry and, as an unexpected side effect, it now matches better the collision layer.
