Issue #28891: Tessa's Fist no longer on her armor

 
I got Tessa's Power Armor, but the Legendary Item Tessa's Fist was NOT on the frame, or in her inventory.

This item is generated at run time by the quest DN109, which starts when the player gets into Quincy location for the first time. In its startup fragment, DN109 sets two stages on the custom item quest to generate Tessa's fist and Clint's laser gun. Please have a look at ticket #28982 which explains in detail how this works.

While testing my fixes for 28982 (with debug messages added to the modified scripts that tell me when they start doing stuff), there was one occasion where both Tessa's fist and Clint's gun were totally missing. My logs showed that the custom item quest was never called, and it turned out that DN109 had not been started as intended. At the time being, I have no clue why this happened. All I can say is that it is apparently a rare bug because I could not reproduce the issue: when re-starting from a save made before I went to Quincy and replaying that part of the game several times, DN109 always started running as expected.
