This issue should have been fixed by Bethesda's official patch years ago (back in the original Skyrim days). If it is an actual issue on PS4, there's nothing we would be able do about it due to Sony's asset restrictions, which don't allow us to modify quest scripts.
The only thing I know to suggest is to revert to a save prior to it happening or to start a new game and hope it doesn't happen again.
