Side note - I see this location is listed on the wiki as prone to failure if the location has previously been cleared; but I have not been to this location, prior to getting the radiant, in this savegame, let alone cleared it.
UPDATE: OK, this is "MinRecruit07" so i was able to get additional detail, which is very interesting - the game's opinion about this quest seems to contradict itself. (wth is stage 150 and how did it get there??)

..... the hey? So it knows the stage is done but also considers it active... and how the heck did it get to 150 from 20, whatever 150 is...? Last but not least, why does sqs list 10, 20, 40..... 30 is supposed to be "report success to settler." It's MIA.
Wiser heads than mine shall have to prevail.
(I was able to successfully setstage 200 on this quest and boot it in the arse, but I've still no idea how it got here...)

Are you able to share a copy of the save that is affected with that bug? Objective 20 is not the same as stage 20, and stage 200 is what completes and clears objective 20. The most relevant data here is the contents of the GhoulBosses alias and the status of those references in the world. If all NPCs in the GhoulBosses alias are dead, the quest is meant to progress. My guess is that, somehow, you have an NPC in that alias that is alive but disabled and as a result has prevented the quest from proceeding. Having a copy of the quest might allow me to investigate the actual cause and how that might have come about in the first place.
For context, stage 150 is a silent stage that shows the boss markers. It almost always gets enabled by default because there are very few quest targets with enough bosses to warrant hiding them. It's mostly just Bethesda accounting for mod weirdness. Stage 200 is set when every NPC in the GhoulBosses reference collection alias is dead, and that is what progresses the quest, completes objective 20, and tells you to return to the settlement.

I'll see if I still have the game this is in, but I have a different PC now and while i transferred a bunch of save data over, i've no idea what playthrough this was from. I'll see if I have this quest outstanding in any current games.
