In Lifting the Vale quest, I found that if you miss obtaining the Akaviri Orders, Akaviri Commander Mishaxhi's dialogue becomes bugged. If you attempt to talk to him without the Akaviri orders, all three dialogue choices result in him seemingly getting mad at the player. Yet, he does nothing. If you attempt to talk to him, his audio bugs out and is that of a regular imperial - as opposed to his intended ghostly voice. When you talk to him after this, you are greeted with a completely empty dialogue menu, which should never occur in the game (as far as I'm aware). It seems that after trying to talk to him initially, his intended dialogue choices never return, causing the player to always be greeted with an empty dialogue menu. You can still kill him, causing the hidden door/wall to open. This does allow you to obtain the Draconian Madstone.
I wanted to attempt the quest without fighting him, so I loaded up my save from before when I talked to him at all and obtained the orders, which did allow me to successfully show him the orders. After this, I received a new quest update. This caused him to disappear and open the hidden door/wall, as intended. After obtaining the quest item (the Draconian Madstone), I received another quest update. However, upon trying to leave the Mouth of the Serpent dungeon, I was greeted with a quest update that I was supposed to receive when I first entered the dungeon. This quest update mentioned how the area was crawling with undead soldiers, which was no longer true. Not sure if this specific bug (receiving quest updates out of order) is related to initially missing the Akaviri Orders.
I also want to note that you are supposed to get a quest update upon getting close to the skeletal body that is next to the Akaviri Orders. However, upon checking my quest progress, I do not seem to have the relevant entry/quest update. So, it seems that I never received it.
I marked the severity as "High" due to these issues causing the quest to be very frustrating. Many people who encounter this may very possible think that the quest is impossible to complete at this point.
