Issue #27725: Doc Weathers' [and others caravans] guard(s) are naked - well know problem

That's a common bug caused by how the NPCs are programmed to wear armor and clothing. Their AI makes them automatically equip armor if doing that would boost their overall damage/energy resistance. Most of the time the NPCs who spawn with armor are also given clothing that the armor can be worn over, so they equip both.
But sometimes the randomized loot lists cause the NPCs to spawn with clothing that cannot also be worn with certain armor pieces. Since the armor pieces typically provide more resistance than the clothing, the AI causes them to equip the armor and leave the clothing in their inventory.

Below is probably the correct detailed explanation;

it happens bc the guards were athletic clothes which don’t support leg armor but they still have a complete set of light leather armor. so when they equip the legs it unequips the clothes. not sure what causes them to equip the legs though.

Maybe it would help to make list of clothing which allows armor over it, instead using the list of all clothing for this?
