Not a navmesh issue or a door issue
Does not effect dogs or humanoid followers
With 3 followers Serana, a modded follower, and a dog all following ..... only Serana doesn't exit
In the same save Serana will exit through other autoload doors so it is not a autoload door issue.
Broken Oar Grotto has a red trigger box covering the exterior door and some Xmarkers inside and out

If you add Serana using Nether's Follower Framework (add to framework) and then use the "snap out of it" command, Serana exits properly

If you add Serana using Nether's Follower Framework (add to framework) and then use the "snap out of it" command and then remover her from the Nether's Framework (remove framework), Serana Does Not exit properly and Nethers Framework is stuck in a half state with Serana removed from the framework but still partially part of it.

In the grotto Serana is being controlled by a Dawnguard package in the save with this issue
Serana has different follow packages depending on where in the Dawnguard Quest you are .... later in the quest you cannot dismiss her, early you can.... this explains why some people have this bug and some don't.

Best guess is that Broken Oar Grotto attaches a script to followers and the Dawnguard package that controls her later in the quest conflicts..
As Serana cannot be dismissed in later parts of Dawnguard ..... she cannot exit because she cannot be dismissed

This isn't a vanilla game issue as she will exit that dungeon just fine regardless of what stage the Dawnguard DLC is at. I've tested it on a save where she's forced to follow you as part of the quest and she exits that dungeon just fine. IMO this is an issue with the follower framework you're using. Not all of those are built properly and it sounds very much like this one isn't if it's letting you cut her loose like that.
