Issue #31501: Vault 118 Elevator - Endless Load Screen

I am sure you have heard of the endless load screen at Vault 118 when exiting the vault. This seems to be happening to players that know the quest and do only what's needed to complete it. I can duplicate this 9/10 times by doing the following:

1. Head in and do the normal investigate the crime scene after you talk to Maxwell and he tells everybody to be quiet and let me do my job.
2. Inspect the blood, the paint can., the bat, then the blood again, to realize it's paint.
3. Talk to Maxwell again.
4. Go directly to the room with the key for the Overseer's door. Do not go to the two markers to talk to the other witnesses, just ignore them.
5. Go directly to the Overseer's office, eject and listen completely to the holotape about Ezra.
6. It will put a marker on Julianna (Ezra). Go talk to Julianna, exhaust the dialog tree, then accuse of murder.
7. Do not let Ezra go, and he'll attack. Kill, then loot him.
8. Brain Dead 2 hasn't started yet. Which is to deal with Ezra or kill him. Go talk to Maxwell, and Brain Dead 2 will start even though you already killed him.
9. Try to exit the vault.

It seems that Brain Dead 2's trigger will not properly start when it's supposed to unless you go through the whole detective deal and talk to everyone. Could this leave a hanging/residual script preventing the player from leaving the vault? My thoughts are that this script should start immediately after the conversation with Julianna/Ezra goes sideways and you accuse him, then forced to make a decision.

Actually none of us has ever heard of this issue.

Unfortunately it's also not something we can address since it's dealing with the transition phase of a loading screen.
