Issue #32481: Restricted outfits for pre-gen settlers.

 
With UFO4 mod installed, all pre-generated settlers in the game world (those at Tenpines Bluff, Somerville Place, Greentop Nursery and Warwick Homestead) are all wearing either Radstag Hide Outfit or Explorer Outfit, instead of the larger pool of outfits available to newly spawned settlers. When tested without UFO4 installed, those settlers came equipped with the expected greater variation of outfits.
Testing (all tests conducted on a new games, both before leaving the vault, after leaving the vault, and after talking with Codsworth)

At each location check each settler (named and unnamed) for what outfit they are wearing.
With UFO4 mod: all settlers wearing either Radstag Hide or Explorer outfit
Without UFO4 mod: all settlers (named or unnamed) wear a variety of different outfits.

If UFO4 is edited to revert the changes made by 27802, the pre-existing settlers start drawing outfits from the expected larger pool. It appears 27802 has uncovered a different bug in Vanilla.

I don't think the leveled items in the outfits mentioned by 27802 are intended to produce a "variety" of outfits. There is an important distinction: Armor_Wastelander_01_GlovesA (Radstag Hide Outfit), Armor_Wastelander_02_NoHood_GlovesB (Scavenger Outfit), and Armor_Wastelander_02_Hood_GlovesB (Explorer Outfit) all prevent armor pieces from being worn over them, whereas the clothing items in the leveled list LL_Clothing_Settler_Underwear (Athletic Outfit, Undershirt & Jeans, and Flannel Shirt and Jeans) all allow a full set of armor to be worn over them.

More likely, the "underwear" clothing was intended to provide a fallback in case the player wants to equip the settler with armor pieces, but since that ends up as the second item in the "outfit" list (and there doesn't seem to be a way to re-order the list), it gets worn instead of the radstag hide or explorer outfit.

As far as a "greater variety of outfits", it doesn't seem that way to me - maybe 3 from that leveled instead of mostly 2 (according to FO4Edit references there are 78 NPCs who wear the outfit that gives the Radstag Hide Outfit, 24 who wear the outfit that gives the Scavenger Outfit, and only 7 who get the Explorer Outfit). By disabling UFO4P, the settlers you mention randomly wear the Athletic Outfit, Undershirt & Jeans, or Flannel Shirt and Jeans instead. Checking Tenpines Bluff, Greentop Nursery, Somerville Place, Warwick Homestead, Finch Farm, The Slog, County Crossing, and Oberland Station, I only saw one adult wearing a different outfit beyond those three: Roger Warwick, because he uses WastelanderOutfit [OTFT:001C269A] instead of one of the three outfits mentioned in the other bug.

Where there are 8 corpses of Railroad Agents.
Vanilla without UFO4: all eight have a random selection of Athletic Outfit, Flannel Shirt and Jeans and Undershirt & Jeans (worn). They also all have Radstag Hide Outfits in thier inventory (not equipped). (The Radstag Hide Outfits would appear to be a bug)
Vanilla with UFO4: all eight only have Radstag Hide Outfits (worn). (This would also appear to be a bug)
It really starts becomig obvious once you start using any mod that adds clothing to the lists (Eli's Armour Compendium for my testing).
Without UFO4, settlers and deceased railroad agents pull from the Athletic/Flannel Shirt and Jeans/Undershirt & Jeans/something from Eli's equipped (and once again Radstag Hide/Explorers/Surveyors oufits in thier inventories).
With UFO4 its Radstag Hide Outfits, Surveyors Outfits or Explorers outfits only (unless you are a dead railroad agent, in which case its Radstag Hide only, it seems).

7 out of 20 use clearly non-randomized outfits, 10 out of 20 use outfits that might have been intended as mostly non-random (Wastelander01/02/02WithHood, and WorkerSlogOutfit02), and only 3 of those 20 use the deliberately random "WastelanderOutfit": Abraham Finch, Roger Warwick, and WorkerSlog1 "Worker" [NPC_:000F8F28]. If I just look at the different outfits used by settlers, ignoring how many NPCs use them, the leveled "underwear" items added to Wastelander01, Wastelander02, and Wastelander02WithHood would seem to be the exception more than the rule. After this investigation, I'm not so sure 27802 should be reverted (though I'm not the one with the final say on that).

As far as the corpses at Kendall Hospital, those are Railroad Agent corpses, not pre-gen settlers, so that might be a separate bug to file. IMO, it would make more sense for those corpses to be using RailroadHQOutfit [OTFT:001344E9] as their default outfit instead of Wastelander01 [OTFT:00021842]
