Issue #37268: Adoring Fan can disappear from the game

 
The Adoring Fan will also disappear from the game if you have him wait in a dungeon and let 3 days pass. After 3 days, all the NPCs in a standard dungeon reset. This will remove the Adoring Fan from the game, and he will not respawn. This applies to most dungeons that are non-quest related and/or inhabited by many enemies.
In Oblivion Remastered
The Adoring Fan will continue to wait for you in the same spot you left him.
